| Transfer of Ownership Form | Due to stricter medical records control, and in the interest of your privacy, we are now asking that pet owners fill sign this transfer of ownership when a pet has been given away so that we may keep your pet's medical records and your personal information as protected as possible.

| Transfer Request For Medical Records to Highland | The updated Indiana Veterinary Practice Act mandates that a request for the transfer of a pet's medical record must now be made in writing. Please supply your previous veterinarian with this document by fax or in person to ensure the timely transfer of your medical record to Highland Vet Clinic

Please fill out this form and return it or fax it to us so that we can ensure your pet's medical records are transferred in a timely manner, and that his/her treatment remains uninterrupted. Non medical records (the transfer of a vaccine only record) to a boarding or grooming facility does not require a form--just a phone call.
